FAMILY CAREGIVER ALLIANCE - A non-profit that has been serving family caregivers for over 40 years. FCA provides online information and education through fact sheets, webinars, and videos. FCA works locally with caregivers of individuals with adult-onset cognitive or neurological impairments by providing assessment, care planning, and ongoing support.

ALZHEIMER'S ASSOCIATION - The Alzheimer's Association leads the way to end Alzheimer's and all other dementia — by accelerating global research, driving risk reduction and early detection, and maximizing quality care and support. SARATOGA AREA SENIOR COORDINATING COUNCIL, SENIOR CENTER - SASCC is a non-profit 501(c)3 organization founded in 1979 to serve the needs of local seniors. SASCC's Saratoga Senior Center provides social events, Wednesday Luncheons, social groups, adult day programs, classes, and services that continue to enrich the lives of seniors and their families.

HEARTS & MINDS ACTIVITY CENTER - Hearts & Minds Activity Center enriches the lives of people affected by Alzheimer's disease and dementia by providing safe, nurturing, intergenerational services and support at our active, large capacity adult day care for persons with all types and levels of dementia. We also provide caregiver education, support, and respite as well as intergenerational services through our onsite preschool for children ages 2-5.

YU-AI KAI - Yu-Ai Kai, founded in 1974 by San Jose State University students, is a non-profit senior community center located in San Jose’s Japantown, that serves 3,000 seniors and their families annually. We provide an environment rich in culture, caring, and enthusiasm to enhance and maintain an independent lifestyle for the growing senior population. Service programing includes Senior Nutrition Program, Social Services, Wellness, Senior Day Services, Caregiver and Grief Support Groups, and Activities. Our services are tailored to support the lives of every generation, and we continually strive to provide services appropriate for today’s families in addition to being a place where people look forward to coming every day.


Santa Clara Family Health Plan - Santa Clara Family Health Plan (SCFHP) is a local, community-based health plan dedicated to creating opportunities for better health and wellness for all.
